Regenerated from the soil seed bank for yellow-flowered sweetclover in 2003. Fewer seedlings occurred for yellow-flowered sweetclover than black medic in 2004, but over 3000 seedlings m 2 occurred for both species in that year. Only around 170 yellow-flowered sweetclover seedlings m 2 occurred in 2002. Fewer seedlings occurred in birdsfoot trefoil than black medic plots in all 3 yr, but seedling numbers always exceeded 200 seedlings m 2 for both species. More than 200 seedlings m 2 also occurred for alsike clover Trifolium hybridum L. ; , kura clover Trifolium ambiguum Bieb. ; , red clover Trifolium pratense L. ; , and white-flowered sweetclover Melilotus alba Medik. ; in 2 yr and in 1 yr for white clover Trifolium repens L. ; . Balansa clover Trifolium michelianum Savi ; showed limited ability to regenerate from the soil seed bank following the wheat phase. Fewer than 100 seedlings m 2 occurred in either 2003 or 2004. Likewise, fewer than 200 seedlings m 2 occurred for 22 other legume species following the wheat phase included in our study data not presented ; , suggesting that these species have limited potential for regenerating pasture in a pasturewheat rotation. Total forage DM production exceeded 3 Mg ha for birdsfoot trefoil following the wheat phase in 2002 and 2003. Forage DM production was less for yellowflowered sweetclover than birdsfoot trefoil in 2002. However, differences were not detected between the two legume species for forage DM yield in 2003. Likewise, white-flowered sweetclover produced comparable amounts of forage DM compared with birdsfoot trefoil in 2003. White-flowered sweetclover was not included with birdsfoot trefoil and yellow-flowered sweetclover in the experiment that was harvested for forage DM in 2002. Dry conditions in 2004 Fig. 1b ; prevented the three legume species from producing more than 0.5 Mg ha 1 forage DM. Forage DM production was 2.5 Mg ha 1 for black medic in 2002. However, black medic produced 500 kg ha 1 forage DM in both 2003 and 2004. Likewise, alsike clover produced only 500 kg ha 1 forage DM in 2003 and even less in 2004. Forage production was 500 kg ha 1 both years that kura clover was grown. The inability of the two clover species and black medic.
Allegra in Japan, where it was launched in November 2000. Allegra increased market share and became the No. 2 antihistamine worldwide based on sales in 2001. Lovenox Clexane sales benefited from the drug having the broadest range of indications among low-molecular-weight heparins LMWHs ; as well as from the launch of a new indication in the United States in the first quarter of 2001 for prevention of DVT deep vein thrombosis ; in medically ill patients with restricted mobility. Also supporting sales was the increasing use of this drug for treatment of various types of serious heart conditions, increased sales force efforts and the continued effect of positive clinical studies. Taxotere sales rose in 2001 due primarily to the results of several clinical trials in breast, lung and ovarian cancer published in 2001, which differentiated it from other therapies. Sales in the United States rose 43% despite the introduction of generic competition for the compound paclitaxel, a competing product. In Japan, sales rose following our acquisition of exclusive rights for this drug in the country after the end of a co-development and co-marketing agreement with Chugai Pharmaceuticals. Delix Tritace sales were primarily supported by the HOPE Heart Outcomes Prevention Evaluation ; study, which was published in January 2000 in the New England Journal of Medicine, and from its status as the only ACE inhibitor with the indication of prevention of stroke, myocardial infarction and cardiovascular death in patients at risk for cardiovascular events. A follow-up analysis of the HOPE study published in 2001 showing that treatment with Delix Tritace was associated with lower rates of new diagnoses of diabetes in high-risk patients also contributed to the higher sales. Amaryl sales were driven mainly by its status as the only oral treatment for type 2 diabetes with three dosage forms and three indications: monotherapy, combination therapy with insulin and combination therapy with metformin, another oral diabetes drug. Amaryl achieved its highest sales in the United States, and also posted significant sales growth in Japan. Lantus sales rose due to the launch of this long-acting basal insulin in the United States in May 2001 and further expansion in Germany, where it was introduced in June 2000. Lantus captured approximately 20% of the market for new vials dispensed in the long-acting insulin market in the United States following its launch, while Lantus gained more than 30% of the long-acting insulin market in Germany based on sales at the end of the year. Ketek was first launched in Germany in October 2001 and achieved an 8% share of the country's macrolide market within nine weeks of launch. Supporting sales was the position of this brand as the first in a new class of antibiotics called ketolides and its indication for treating respiratory tract infections, including those resistant to certain commonly used antibiotics. Actonel sales were supported by the publication of clinical data supporting the efficacy of this drug in reducing fractures among people with osteoporosis. Actonel also received a new EU indication in 2001 for the reduction of hip fracture risk in women with established postmenopausal osteoporosis. Further supporting sales was the growing acceptance of this drug in the United States, its primary market, as well as market launches in certain European countries. Actonel is being co-developed and co-marketed with Procter & Gamble Pharmaceuticals. Actonel generated combined sales for the two companies of e 309 million in 2001 compared to e 63 million in 2000.

| Non-MOH Supported Clinical Enrichment CME Programs: Several successful major programs have been developed in DFCM. Funding comes from a variety of sources, usually on an individual-by-individual basis. Each program provides a combination of theoretical study, seminars, rounds, etc; research project work; clinical experience and selective graduate-level audit opportunities. Each program is situated in a divisional site or group of sites with particular expertise and interest in the designated field. There is an identified program director working with divisional support and recognition co-ordinated through the DFCM Postgraduate office. Certification is usually provided at the divisional CME level. There have been preliminary discussions with the College of Family Physicians of Canada Dr. Paul Rainsberry ; about future plans to consider a generic accreditation process. Academic Fellowships International Sabbaticals Courses: Initially, many of the FD programs were developed to provide faculty training for new or expanding hospital teaching units divisions and funded from start-up monies. Subsequently, international interest in primary care family medicine has grown and many faculty from around the world have undertaken continuing education within the various programs. The fees from such CME have helped to continue DFCM support of the FD portfolio. All programs have been free to DFCM faculty reduced rates for webbased programs ; . Faculty from at least 16 countries and all continents except Africa have participated in these courses. Providing CME FD courses parallel to the formal graduate studies program [see report of MHSc FCM ; below] enables a rich exchange of knowledge and insight for learners functioning at a variety of career stages and levels from different backgrounds and allows greater use of resources. The full Academic Fellowship Program requires satisfactory completion of 10 units of work in areas of teaching and learning; individual and family development; social, political and scientific issues in Family Medicine; practice teaching, research or creative professional experiences and two elective topics. An internal review of the Academic Fellowship Program was conducted in 1998. It recommended the Academic Fellowship Program be tuned to the development of leaders in Family Medicine, particularly at the international level. Health professionals from other faculties Nutrition, Nursing, etc. ; and from the Royal College Specialties have taken a variety of the CME courses and workshops available approximately nine in the last three years ; as personal faculty development. Funding from the Dean's Excellence Fund in 2001 and 2003 enabled DFCM to establish a CE Clinical Teacher Certificate of Completion practical training program, comprised of 4 units of work. These are the required courses Teaching and Learning A and lenalidomide.
Alcohol restriction Nine guidelines JNC, VHA, BHS, CMA, WHO, SA, SIGN, ISCI, ESH ; quantified the recommended daily or weekly intake of alcohol for hypertensive patients. In four this was expressed as an amount of ethanol. Three SA, NZ, CMA ; described `standard drinks'. These terms may be difficult to incorporate into the clinical setting without interpretation. The JNC guideline estimated that limiting alcohol consumption can lower systolic blood pressure by 2-4 mm Hg.
Discussion In the present study, using specific radioimmunoassay for 21 amino acid ET, it was possible to evaluate the concentrations of active ET in FF relation to ovarian physiology. The primary antibody used in this method fully crossreacted with all three isoforms of ET, and therefore values presented in the present report refer to the total amount of active ET, but not to their weakly active precursors. ETA receptors are predominant in the human ovary; however, most of them are localized in the theca interna and on intra-ovarian blood vessels Mancina et al., 1997 ; , and are thought to participate in mitogenic and vasoregulatory effects of ET Kamada et al., 1995 ; . All data, including serum hormone measurements, pituitary volumes, and relative signal intensities, were reported as means SDs. The paired Student t test was used to evaluate the difference in pituitary volumes and relative signal intensity ratios in each subject between the euthyroid state and the hypothyroid state. The paired Student t test was also used to evaluate the difference in pituitary volumes and relative signal intensity ratios in each healthy volunteer between the two MR images obtained with an interval of 5 weeks. In each analysis, a P value of .05 was considered to indicate a significant difference.
Experimental Basis for Treating Autoimmune Diseases with Bone Marrow Transplantation D.W. van Bekkum Comprehensive Cancer Center Rotterdam, Rotterdam, The Netherlands The manifold animal models of autoimmune diseases AIDs ; can be distinguished in hereditary and induced AID. The inherited types develop spontaneously with high frequency in specific inbred strains of rodents, but external factors such as diet and microflora may influence age of onset and incidence. The induced types require some type of immunization. Susceptibility or resistance to induction is strain-specific and therefore genetically determined. Many of the diseases in both categories can be transferred to healthy animals by grafts of bone marrow or lymphoid cells and, conversely, the diseases may be prevented by grafting cells from resistant donors into susceptible animals [1]. The logical next step was to investigate whether animals suffering from overt AID respond to transplantation of bone marrow from allogeneic-resistant donors, which, of course, requires myeloablative conditioning. One of the problems with allogeneic bone marrow transplantation BMT ; is the occurrence of graft-versus-host disease GVHD ; which, particularly in its chronic form, has many characteristics in common with AID. This complication was circumvented by Ikehara et al. [2] who were the first to cure fully developed AID in BXSB and in MLR lpr mice with T celldepleted allogeneic BM. In the MLR lpr mice the disease recurred coincidentally with a reversal of the chimeric state. Recently, Karussis et al. [3] reported effective prevention of the development of disease manifestations in MLR lpr mice by treatment with high-dose TBI or cyclophosphamide CY ; and rescue with syngeneic BM. CY proved more effective than TBI. Furthermore, T cell depletion of the BM graft produced better results than did unmanipulated marrow. In this experimental set-up, syngeneic marrow is derived from donors with an autoimmune-prone condition similar to the recipients, and is therefore equivalent with autologous marrow. Similarly, induced adjuvant arthritis in rats [4] and induced encephalitis EAE ; in rats [5] responded with complete and lasting remission to transplantation of allogeneic BMT from resistant donors. Surprisingly, van Bekkum et al. found syngeneic from the same susceptible inbred rat strain as the recipients ; and autologous BM equally effective in inducing complete remissions as allogeneic BM in both the adjuvant arthritis [6] and the EAE article submitted ; . Already in 1983, Prestonk et al. [7] reported complete recovery of rats with experimental autoimmune myasthenia gravis EAMG ; following combined conditioning with TBI and CY, and autologous BMT. In adjuvant arthritis, spontaneous or induced following reimmunization ; relapses are rare events, even following autologous BMT, but in the EAE model the incidence of both types of relapses is higher with autologous marrow grafts. In the EAMG rats, spontaneous relapses did not occur, and reimmunization of the cured animals resulted in similar reactions as seen after primary induction. In the EAE model, relapses were shown to be dependent on residual autoimmune T lymphocytes in the recipient as well as on the reintroduction of autoimmune T cells with the autograft [to be published and 8]. The latter observation provides a.

Leading to the production of a range of proinflammatory cytokines and the orchestration of other signalling pathways. It has been claimed that adenovirus infection, especially at high multiplicities, can lead to the activation of NF-B at early stages of infection Clesham et al., 1998 ; Lieber et al., 1998 ; . One mechanism for achieving this could be by the binding of E1A to the p65 subunit of NF-B, although it is also apparent that this activation can be suppressed by E1B 19K Pahl et al., 1996 ; Schmitz et al., 1996 ; . Reference has been made above to the very early induction, possibly by the interaction of the penton base in the virus with cellular integrins, of the Raf\MAPK and other pathways. These may play a role in the activation of NF-B Ghoda et al., 1997 ; as well as in the early release of chemokines Kuhnel et al., 2000 ; Muruve et al., 1999 ; and interferons, which are important components of the innate response to infection. However, the induction of these endogenous genes following infection appears to be quite cell dependent, with human endothelial cells displaying a range of signalling molecules at 24 h, a scenario not seen in human dermal fibroblasts or alveolar macrophages Ramalingam et al., 1999 and lavender.
Type 2 DiabetesAdult see Table 3 ; . In large, randomized, controlled clinical study Study E ; n 570 ; , LANTUS was evaluated for 52 weeks as part of a regimen of combination therapy with insulin and oral antidiabetes agents a sulfonylurea, metformin, acarbose, or combinations of these drugs ; . LANTUS administered once daily at bedtime was as effective as NPH human insulin administered once daily at bedtime in reducing glycohemoglobin and fasting glucose. There was a low rate of hypoglycemia that was similar in LANTUS and NPH human insulin treated patients. In a large, randomized, controlled clinical study Study F ; , in patients with type 2 diabetes not using oral antidiabetes agents n 518 ; , a basal-bolus regimen of LANTUS once daily at bedtime or NPH human insulin administered once or twice daily was evaluated for 28 weeks. Regular human insulin was used before meals as needed. LANTUS had similar effectiveness as either once- or twice-daily NPH human insulin in reducing glycohemoglobin and fasting glucose with a similar incidence of hypoglycemia.
