Addison's disease is a rare disease that occurs when the body produces too little of the hormones cortisol and aldosterone. Both hormones are important for normal body function and are made by the small glands located just above the kidneys adrenal glands ; . Cortisol affects almost every organ in the body. The adrenal glands release extra amounts of cortisol when the body is under stress due to illness, injury, surgery, childbirth, or other reasons. Extra cortisol helps the body cope with that stress.

Somatostatin is a small neuropeptide that is produced in the central nervous system, where it acts as a neurotransmitter. This neuropeptide also has an important role in the neuroendocrine system as an inhibitor of hormone release, the most striking example of which is the inhibition of growth hormone release. The role of somatostatin in eye disease recently became of interest because of its role in proliferative diabetic retinopathy and cystoid macular oedema 13 ; . In these two diseases a biopsy displays subepithelial bullae, and direct immunofluorescence demonstrates autoantibodies against the basement membrane of the surface epithelia. Erythema multiforme Bean and Quezada, 1983 ; is a heterogeneous symptom complex characterized by pleomorphic skin lesions that are often symmetric. The most characteristic skin lesion is an iris, or target, lesion; this consists of concentric lines of erythema interspersed with normal skin, the center of which is usually a papule. This is frequently associated with a systemic symptomatology such as fever, malaise, and arthritis; when it involves the mucous membranes it is frequently called the Stevens-Johnson syndrome Plate 11, B ; . The etiology of this disease is multifactorial. It can be seen after streptococcal illness and infections such as herpes and mycoplasma. Penicillin and sulfa are the most common drugs implicated in its etiology. Occasionally patients with a splenectomy develop recurrent erythema multiforme, but it has an unknown cause in 50% of the patients. Oral manifestations of erythema multiforme are frequently estimated at around 50% and consist of bullae, ulcerations, and hemorrhagic crusts, particularly around the lips. Histologically, oral lesions exhibit degenerative changes of the epithelium, with epithelial vacuolation of the basal layer and inflammation mostly with mononuclear cells. Associated with such changes are deposits of immunoglobulin and complement, which have been found in subepithelial vessels. Systemic steroids are indicated for very symptomatic cases. Lichen planus Pedersen and Klausen, 1984 ; is a dermatologic condition that affects the mucous membranes of the lips, tongue, and tonsils as the sole manifestation in one third of the cases, dual involvement of the mucous membranes and skin in another third, and the skin alone in another third. The mucous membrane lesions are asymptomatic papules that range in color from white to blue to gray and whose morphology may be reticular, linear, annular, or even plaquelike Plate 12, A ; . Occasionally, bullous lesions are seen that seem to indicate mucous membrane pemphigoid. The skin lesions are violaceous papules on the extensor surface of the forearms and neck, which are chronic and heal with some atrophy. The pathogenesis of this disease is far from clear but is thought to be an immunologic disease because of reports of fibrin and complement deposition in the basement membrane zone with an infiltration of inflammatory cells just below the dermis. That this disease can be seen after bone marrow transplantation and in association with SLE supports this hypothesis. Findings indicate that a combined type IV delayed ; and III antigen-antibody ; hypersensitivity reaction takes place in the lesions in response to locally produced or released epidermal mucosal antigens and pentasa!

Summary judgment. The Court reaches this conclusion because it is clear that the most serious matters about which Plaintiff complains, i.e., that he was denied surgery for his various ailments, amount to disagreements over treatment, which are not actionable as constitutional violations. To illustrate that commitment we present case studies of three recent purchasing decisions and examples of our ongoing relationship with the City's main computer workstation vendor. In the first procurement example, the committee responsible for standardizing on a laptop platform used a list of environmental questions as a part of the vendor evaluations. They asked questions about toxic materials used in manufacturing and contained in the products themselves. They also asked about end of life recycling options and packaging. A list of the questions used is available. Vendors were informed that the environmental considerations would be given equal weight with the features and cost of their products. This provided vendors and the City with a new paradigm in IT purchasing. It gave the City and its vendors an opportunity to consider the whole life of their products and how they affected the planet and those who live on it. Randomized study of IFN + DTIC versus DTIC or IFN alone failed to confirm an advantage of combination over single agent DTIC. Combining chemotherapy with IL-2 and IFN is commonly referred to as biochemotherapy and has been reported to induce high response rates and long-term survival. Combined modality therapies that employ a high-dose IL-2 backbone are given in hospital where the necessary services to support these individuals are available. Recent enthusiasm for bio-chemotherapy has been based on success reported from single institution, single arm trials and precipitated a number of larger single institution and multi-center randomized studies comparing different variations of bio-chemotherapy with combination chemotherapy.Two studies NCI and the Italian Group ; failed to find any statistically significant improvement in response rates or survival. The third study MD Anderson ; demonstrated a doubling of objective response rates 48 versus 25%; p 0.001 ; and of time to progression of disease 4.9 versus 2.4 months; p 0.008 ; , but no advantage for overall median survival 18.7 versus 15.4 months; p 0.99 ; .These findings generated renewed enthusiasm for biochemotherapy. Subsequent studies have now been completed. The US intergroup study compared IL-2 IFN + CVD with CVD, in 397 patients. Primary end-points included response, survival, and time to progression TTP ; .
