Abstract. Chloroquine combined with primaquine was evaluated for therapy of uncomplicated malaria caused by Plasmodium falciparum in nonimmune Javanese migrants to northeastern Papua, Indonesia. Subjects were randomized to treatment with standard chloroquine therapy 25 mg kg in 3 doses over the course of 48 hours ; with 30 mg primaquine administered daily for 28 days n 25 ; or placebo of primaquine n 28 ; . The 14-day cumulative incidence of therapeutic failure was 56% with primaquine and 79% with placebo odds ratio [OR], 0.35; 95% confidence interval [CI], 0.11.3; P 0.08 ; . Primaquine administered daily created a marginally significant improvement in therapeutic efficacy at day 14, but not at day 7 20% versus 36%; OR, 0.2; 95% CI, 0.11.8; P 0.2 ; or day 28 82% versus 93%; OR, 0.31; 95% CI, 0.042.1; P 0.23 ; . This report corroborates studies suggesting that therapeutic doses of primaquine exert no discernible effect on parasitemia by P. falciparum. INTRODUCTION Chloroquine and primaquine remain in common use against uncomplicated malaria in the developing world. Chloroquine still constitutes first-line therapy for infection by Plasmodium falciparum for most people exposed to risk in Asia--for example, in India, the Philippines, and Indonesia. The very poor virological responses observed in studies using ABC 3TC TDF or ddI 3TC TDF might be explained by the selection of a pre-existing viral population resistant to one or more drugs included in the regimen. Genotypic analysis of HIV isolates obtained after failure of ABC 3TC TDF or ddI 3TC TDF show a very high prevalence of isolates with single M184V I ; or double M184V I and K65R ; reverse transcriptase mutations Table 2 ; . If pre-existing double mutants are the main reason for failure of the ABC 3TC TDF and ddI 3TC TDF combinations, it is somewhat difficult to explain why a number of isolates obtained after virological failure only contain M184V I without K65R. This discrepancy might be related to factors that can diminish the sensitivity of genotypic testing, such as minority viral populations, analytical methods and fitness of the mutant viruses. It appears that pathways to resistance to triple nucleoside regimens seem to be very different for regimens that include a thymidine analogue. In the ACTG5095 study, 17 none of the 81 samples taken from patients who experienced virological failure after treatment with ABC ZDV 3TC contained the K65R mutation, while 36 92% of samples in patients failing ABC 3TC TDF or ddI 3TC TDF were positive for K65R Table 2 ; . Interestingly, while thymidine analogue mutations TAMs ; were seen in 12% of patients failing ABC ZDV 3TC therapy in the ACTG5095 study, TAMs were almost never seen in patients failing ABC 3TC TDF or ddI 3TC TDF regimens Table 2 ; . A recent study of the ABC 3TC TDF and ZDV combination further supports the notion of antagonism between TAMs and the K65R mutation. The COL40263 study was a pilot, openlabel, multicentre study evaluating the efficacy and safety of once-daily ABC ZDV 3TC plus TDF in antiretroviral-naive individuals.32 As a result of the poor response previously reported with once-daily TDF plus ABC 3TC in the ESS30009 study, a planned interim analysis was carried out on 88 subjects with at least 8 weeks of data and 54 individuals who completed 24 weeks of therapy. An observed analysis of the 24 week data showed that 79% achieved a viral load of 400 copies mL and 67% a viral load of 50 copies mL. For the 35 individuals who had a high baseline HIV viral load of 100 000 copies mL, 74% had 400 copies mL and 60% had 50 copies mL. These results are better than those reported in the ESS3009 study. From eight patients with evaluable genotypes, six had wild-type virus and two had mutant virus K103N and 215T A reversion mutation ; . At withdrawal or at the last post-week-24 visit, two had wildtype virus, two had 1 TAM without M184V, and three had 1 TAM with M184V. Only one patient had the K65R mutation. Investigators from VircoLab, Inc.33 have found a strong negative relationship between K65R and TAMs 41L, 67N, 210W and 215Y F ; . When K65R was added to two different clones 41L 210W 215Y or 67N 70R 215F ; , ZDV resistance was reduced 10-fold. In addition, TAMs altered the phenotypic effect of the K65R mutation, reducing resistance to zalcitabine, ddI, d4T, ABC and TDF. Finally, a recent retrospective analysis has shown that individuals receiving a thymidine analogue were significantly less likely to develop the K65R mutation as were individuals with pre-existing TAMs.34 Taken together, these results clearly suggest an antagonism between the development of K65R and TAMs and primidone.
